There are several required forms for registration and enrollment:

• Registration form must be picked up at the Stepping Stones office.

• Emergency Care Information form may be printed (click here for a printable copy), or picked up at the Stepping Stones office.

• Health Form may be printed (click here for a printable copy), or picked up at the Stepping Stones office.

• Medical Administration form may be printed (click here for a printablecopy), or picked up at the Stepping Stones office.

• A copy of your child’s Birth Certificate OR Passport.
